We are seeking a dedicated Field Nurse to join our healthcare team. The Field Nurse will be responsible for providing high-quality medical care to patients in various settings, ensuring their well-being and comfort. We are looking for field nurses in Monmouth County and surrounding areas.

Qualifications:

· A licensed RN in good standing, issued by the NJ Board of Nursing

· Must have one-year professional experience as a registered nurse

· Home Care experience preferred

· Must pass the Home Care test.

· Availability via telephone and to provide home visits

· Bilingual in Spanish preferred

Responsibilities:

· Provides nursing care for assigned patients using the nursing process according to the standards of the nurse practice act.

· Provides guidance to assign appropriate CHHA’s and skilled personnel to meet the client’s needs.

· Conducts initial patient assessments and ongoing reassessments and documents accurately

· Develops patient’s plan of care according to needs identified.

· Supervises and directs field personnel in providing care.

· Coordinates with community agencies to provide for continuity of care.

· Demonstrates the ability to communicate effectively with the client, client family members, other members of the health care team and staff of the agency.

· Interprets to the client and family implications of the client’s health status.

· Consistently adheres to standard precautions, aseptic technique and infection control guidelines.
