Join the Adult Home Health team !

Our Community Health Nurses provide skilled, compassionate care to patients in their homes—helping individuals remain where they feel most comfortable 🏡. We work to the top of our licensure and collaborate closely within a comprehensive, interdisciplinary team that includes:

• Registered Nurses (RNs) 🩺

• Licensed Practical Nurses (LPNs)

• Licensed Nursing Assistants (LNAs)

• Physical Therapists (PTs)

• Speech-Language Pathologists (SLPs)

• Social Workers

• Dedicated Administrative Support

Together, we ensure each patient receives the highest level of personalized, at-home care.

• 1+ year of home health or public health nursing experience preferred, OR a combination of education and experience that provides comparable skills

• Home care experience is helpful but not required —we will happily train motivated candidates who are eager to learn 🌟

• $10,000 Sign-On Bonus for external applicants (prorated for part-time roles) 🎉

• Mileage reimbursed at federal gas rates 🚗💨

• ​On-call: Not required 🙌

Requirements:

• Current RN licensure recognized by the State of Vermont required.

• Appropriate experience in specific clinical area. Varies by unit.
