Thrive On Care is seeking skilled and reliable Home Health Aide with proven in-home care experience to support adults with disabilities and aging seniors in the San Antonio and surrounding areas. Responsibilities

• Assist clients with daily personal care tasks like bathing, grooming, and dressing
• Provide safe mobility support and transfers using proper equipment (e.g., gait belts, Hoyer lifts)
• Create meaningful companionship through conversation, activities, and engagement
• Offer timely medication reminders in alignment with each client’s care plan
• Handle light household tasks: meal prep, laundry, and tidying
• Transport or accompany clients to errands or appointments (reimbursement provided if using own vehicle)
• Identify and report changes in health, behavior, or environment to the office team
• Follow established care plans and maintain clear, respectful communication
• Uphold a safe and clean in-home environment

Qualifications

• Minimum of 6 months experience (home care, facilities, or group homes)
• Must be 18 or older
• CNA certification preferred but not required if experienced
• Strong knowledge of personal care and safety practices
• Ability to work independently and follow client-specific instructions
• Valid driver’s license, insured vehicle, and reliable transportation
• Must pass a background check and screening
