As a RN Care Coordinator, you will be a central figure in patient care, seamlessly navigating the healthcare journey to achieve optimal outcomes and an exceptional patient experience.

Responsibilities

Assess, plan, and facilitate comprehensive care across the continuum.

Advocate for patients while collaborating with physicians, nursing, departments, insurers, and post?acute providers to ensure timely, high?quality transitions.

Apply clinical guidelines to ensure progression of care and utilize resource?management tools.

Job Requirements


  • Graduate of an accredited school of nursing.
  • Minimum two (2) years of acute hospital clinical experience or a Masters degree in Case Management or Nursing field in lieu of one (1) year experience.
  • Able to apply clinical guidelines to ensure progression of care.
  • Knowledge of managed care and payer environment preferred.
  • Must have critical thinking and problem?solving skills.
  • Collaborate effectively with multiple stakeholders.
  • Professional communication skills.
  • Understand utilization management and case?management program integration.
  • Team player with ability to assist other members as needed.
  • Thrives in a fast?paced, self?directed environment.
  • Knowledge of CMS standards and requirements.
  • Proficient in prioritizing work and delegating where indicated.
  • Highly organized with excellent time?management skills.


Preferred
  • Bachelors Degree in Nursing (BSN) or related healthcare field.
  • At least five (5) years of nursing experience.


Licensee
  • RN license in the state(s) covered is required.
  • California RN:CA
  • BLS required within 3 months of hiring.



Where Youll Work

Dominican Hospital (a member of Dignity Health) is a 222?bed facility located in Californias Central Coast. The hospital offers comprehensive emergency services, cardiac, orthopedics, oncology, womens and childrens services, and more. It is a Certified Stroke Center and Chest Pain Center, with advanced neurology and endoscopic services.

The hospital has received national recognition for patient safety, cardiac care, and stroke treatment, and serves as a STEMI receiving center with two 24?hour cardiac catheterization labs. Dominican also hosts a level III NICU partnership with Lucile Packard Childrens Hospital Stanford.

Neurosurgery is offered through a partnership with Stanford Health Care. The facility boasts cutting?edge cardiac procedures such as Transcatheter Aortic Valve Replacement (TAVR).
